APPROACHES: Advanced Care Planning

The Advance Care Planning (ACP) Lead Training Program is a comprehensive online course designed to empower nursing home staff with the knowledge and skills needed to facilitate meaningful conversations about goals of care, values, and treatment preferences with residents and their families. This program addresses the critical need for systematic, high-quality advance care planning in long-term care settings, supporting resident-centered care and improving outcomes for patients and families.

Participants will learn to:

  • Engage interdisciplinary teams in ACP discussions
  • Explore and document facility practices and policies
  • Facilitate ACP conversations and meetings with residents and families
  • Track ACP decisions and ensure they are reflected in care plans and medical orders
  • Manage conflict and support families through complex decision-making
  • Understand the roles of various care team members in ACP
  • Educate residents and families about legal representation, artificial nutrition, comfort care, hospitalization, hospice, palliative care, and antibiotic use

The ACP Lead will become the in-house expert and champion for advance care planning, receiving structured training and ongoing resources to support staff, residents, and families.
