Course Description:

The 2023 MDS Updates include a focus on medication reconciliation. Ineffective medication reconciliation can result in unintended consequences. Medication discrepancies are common, and residents in skilled nursing facilities are often prescribed 10 or more medications. The MDS 3.0 includes the provision of current reconciled medications at discharge. Plan to attend this session and discover elements of an effective medication reconciliation process. All attendees will receive a useful tool related to medication reconciliation.
